According to the decision of the Information Management Unit (IMU) of the Ministry of Health and Population, a website and app have been developed for people aged 18 and above who want to be vaccinated against COVID-19 to fill up the pre-registration form themselves. People 18 years of age and older who fill out this voluntary form will be able to get vaccinated based on preference and availability of vaccines.
- This is a voluntary personal application form.
- People 18 years of age and older who have not yet been vaccinated are invited to fill out this form.
- The details obtained from this application form will facilitate the planning and management of immunization at the municipal level.
- Filling out this form does not guarantee vaccination.
