- Passport size photo was taken recently - (1 copy).
- Certificate of Health (Nepal Electricity Authority, Staff Service, Conditions Regulations 2075 (with resources) in the format of Schedule-10.
- Identity Document (Admission Card or Citizenship Certificate or Identity Card).
- Copy of all educational qualification certificates certified by Notary Public - 1/1 copy.
