3) Regarding Examination Form:
A) After completing the examination application form, the student must complete and submit it to the respective office.

E) The campus application must be signed by the Campus Chief, along with each student's application form and photo, before the campus application can be submitted to the campus.
F) A photocopy of the original credit card for the first time given the regular examination for the opportunity examination should be submitted along with the application form.

Note:
